This paperwork is required to enroll in ANY school aged care for the 2026-27 school year at either Pabst Farms YMCA or Schultz Family YMCA. This includes 2026 Summer Camp as well as the 2026-27 School Year care. It only needs to be completed once per child. Please have immunization records ready for upload for all children and the health report for children 4 or younger.
Incomplete or missing paperwork will result in removal from our system and may result in loss of placement. Paperwork reviewed on a first-come, first served basis.
